Change of Address or Name

To change their name, students must complete a Change of Address/Name Form, and submit this form with a copy of a government-issued document indicating the prior name and updated name to the Office of Student Records.

To change their address, students may submit the request through EMPOWER ME or submit a Change of Address/Name Form to the Office of Student Records.
