Student Health Services

Provisions of the services to which a student is entitled are detailed below:

  1. Student Health Portal: Students will be instructed to register for the portal once they register for classes and obtain their West Liberty ID and email address. The required health forms can be completed electronically on the portal. Students who fail to submit the required health forms may be denied student medical care, participation in intercollegiate sports, or entry into some academic programs.
  2. Physical Exam: A physical-is-required for any graduate student wishing to use Student Health Services. The physical exam form must be printed from the Student Health Portal, completed by the student's health care provider, and uploaded to the portal.
  3. Physicians: During the regular university year, a university physician is available for consultations week day mornings by appointment. Physicians are also available at Doctor's Urgent Care of Wheeling during their hours of operation.
  4. Registered Nurses: Evaluation and treatment of students with health concerns will be provided by the Director of Student Health Services by appointment, who maintains regular office hours Monday- Friday 7am-3pm. Most services are free. Student Health Services does not bill health insurance.
